Why this exists and What happens next.

Med Record Vault is a project led by a Philadelphia-based founder and long-time patient.

This project started from personal experience navigating fragmented care and conversations with others doing the same. Before anything gets built, the goal is to understand how people actually manage their medical history today. This page is part of that research.
